Plan Duration
Plan duration is for 1 year from enrollment date and will renew yearly, unless a written cancellation notification is received prior to renewal date or you fail to pay the yearly fee. The plan may be discontinued by Associated Dentists at any time upon written notice to you, at the end of any 1-year term.
Plan Payment Options
One-time yearly payments can be made with cash, check or credit card. All patient payment portions for additional dental services, not included in the preventive care services outlined here, are due at time of service in order to receive the 20% discount provided by the plan.
Interest-free payment plans for additional dental services are available on request with approved credit through Care Credit.
Plan Participation Guidelines
Associated Dentists Care Plan is not dental insurance, but a dental care membership plan provided by Associated Dentists and is not transferable to other dental offices or to any other person or persons except those covered under the specific Membership plan. The VIP membership program is only available to patients who do not have existing dental insurance. It is the patient’s responsibility to schedule and keep their appointments. NO REFUNDS WILL BE PROVIDED FOR DUES PAID , including failure to schedule and maintain appointments.
Please notify our office at least 48 hours in advance if you must change a scheduled appointment. Fees for dental services other than the preventive care services included in the yearly fees may change at any time.
